Anybody else remember the M-16 ammo that caused rifles to fail in the field, costing American lives in Vietnam.  Somebody switched orders to an unauthorized supplier that used a propellant that left heavy residue that quickly built up and jammed the rifles.

Never heard of that one, but it's not surprising. Human greed is only restrained by the humans that experience it. And there are examples throughout history and scripture of those who don't show restraint.
